11/8/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Chevelle upon arrival to the home setting. Upon arrival to the home setting, Chevelle appeared to be in a good mood evident by her facial expressions such as a smile on the face. Counselor informed Chevelle the topic of today’s session. Intervention: QMHPA begin session by inquiring about Chevelle's emotional well-being. QMHPA reiterated Chevelle’s goals discussed the importance of maintaining good mood stability and practicing stress relief techniques when she becomes frustrated. QMHPA reiterated anger management techniques and reviewed stress relief techniques to assist her with controlling and maintaining good mood stability. QMHPA implemented anxiety reduction techniques to assist Chevelle with episodes where in which she becomes anxious or frustrated. QMHPA provided social support and promoted positive and healthy expectations. QMHPA reviewed different levels of stress and provided Chevelle with stress chart to aid her in managing her stress level on a daily basis. QMHPA encouraged Chevelle to utilize chart as this would help her recognize when she has become frustrated and assist her with deescalating down from her highest levels. QMHPA engaged Chevelle in a skills building activity. QMHPA reviewed anger management techniques and instructed Chevelle to be mindful of her stress levels and to utilize learned techniques to assist her with her mood as well. QMHPA discussed deep breathing, counting to 10 as well as muscle relaxation to assist Chevelle with her anger and anxiety. QMHPA instructed Chevelle to discuss and jot down ways in which she could relieve stress as she had learned several ways during previous sessions. Response: Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her by QMHPA. While exploring Chevelle’s current emotional state, she indicated to QMHPA she was in a good mood while discussing with counselor on how she is continuing to search for apartments and not stressing about it because she knows that being patient is going to get her what she needs. Chevelle actively engaged in conversation and was able maintain good mood stability when discussing situations that frustrates her and causes her to present a negative attitude. Chevelle agreed that she becomes anxious during situations as she does not know what to expect however is expecting things to work in her favor. Chevelle stated that she does however recognize the importance of utilizing learned techniques however these techniques go out the window when she faced with the actual situation. Chevelle informed QMHPA that she would work on applying techniques to assist her with her anxiety and her mood when she is faced with situations that frustrated. Plan: QMHPA will continue to assist Chevelle in connecting her feelings to her behavior and consequences (1-2 times weekly).
